Thank you for your interest in being included in our World Mission Services Project Booklet, offered through the Catholic Foundation for the Diocese of Green Bay, Inc. This booklet is used as an optional giving resource by our parishes, schools and interested individuals. We are currently accepting applications for our 2025-2027 booklet. Please complete your application by June 15, 2024.

You can begin to prepare by browsing the current Project Booklet and by reviewing the guidelines below.

Guidelines:

This booklet is used for short-term projects or needs of a village, parish, diocese, or group that has a connection to the Diocese of Green Bay.
Should your project receive a donation, a U.S. mailing address for checks is required. Checks must be made payable to a Diocese, Religious Order or Organization, not an individual.
A written endorsement from the local Bishop, Religious Superior, or Director of the Organization is to be submitted with the application.
Donations are to be used only as designated by the donor.
The project should clearly state its purpose/goals and be specific as to how items donated will be used. A cost estimate (U.S. dollars) is helpful.
Photos 4×6 inches or larger at 300 dpi can be submitted for use on the project page.
Other than a thank you, donors are not to be solicited for further donations. Do not put donor names on any mailing lists.

How To Submit My Application
1. Please click submit after the form is completed.
2. You will automatically be forwarded to a thank you web page.
3. You will receive an email at the address you entered in the form thanking you for completing your application and providing additional instructions. Attached to this email will be a PDF copy of your completed application.
4. If you did not attach the required documents to this application, you MUST send copies of these to World Missions, Catholic Foundation, PO Box 22128, Green Bay, WI 54305-2128, USA or [email protected]. Your application will not be processed until all documents are received. The deadline for the application and all required documents is June 15, 2024.
