How to Donate Stock

Thank you for your interest in making a gift through a donation of stock! Many people choose to donate stock they currently own to benefit their church or Diocese.  The Catholic Foundation is here to help you accomplish making this stock gift. Please complete the form below to initiate your stock gift.  This form helps the Catholic Foundation ensure you receive proper credit for your gift and makes sure we direct it for the purpose you intended. Once you complete the information below and click submit you will receive an email that contain all the information you need to transfer your stock gift.  This includes the Catholic Foundation brokerage name, account name and number and DTC number. Thank you for your gift!

If you do not have email, please call Ken Kolarik at 877-500-3580 x8264 or 920-272-8264 to receive the information needed to make your stock gift.

  • Please be specific with the purpose of the gift along with the approximate dollar amount or number of shares for each purpose this stock gift will benefit.
  • If the stock is held in “street name” at your brokerage firm, shares can be transferred electronically to the Catholic Foundation. Provide your broker with the following:
  • 1. Receiving firm: Associated Investment Services
  • 2. DTC # of Associated Investment Services: #0226
  • 3. Account title: Catholic Foundation for the Diocese of Green Bay, Inc.
  • 4. The final information will be sent in an auto-response email.
  • For other types of stock transfer instructions such as through Computershare, direct through the stock company or if you hold the stock certificates, please contact Ken Kolarik at 877-500-3580 x8264 or 920-272-8264.
